% of Charges - you will be asked to pay the indicated percentage of the provider's normal billed charges.
Medicare's Allowance – you will be asked to pay the applicable Medicare allowance for the services you receive. Medicare allowances are typically lower than insurance company allowances and 25% to 40% lower than usual and customary charges. For services not valued by Medicare you can expect to pay 60% of the provider's usual and customary charge.
80% of Medicare means the provider will ask you to pay 80% of the applicable Medicare allowance
